By requesting a consultation, enrolling in our services, or using our website, you agree to be bound by these Terms and Conditions ("Terms"). Please read them carefully. If you do not agree with any part of these Terms, please do not use our website or services.
1. Services & Eligibility
Pulse Home Health & Hospice provides Medicare-certified home health and hospice services to eligible patients in Southeastern Louisiana. Eligibility for services is determined through a professional assessment and must meet Medicare, Medicaid, or private insurance guidelines, or be arranged through private pay. We reserve the right to accept or decline referrals based on clinical appropriateness, staffing availability, and geographic service area.
2. Payments & Insurance
Most services are covered by Medicare, Medicaid, private insurance, or VA benefits when eligibility criteria are met. Our team will verify coverage and obtain necessary authorizations before starting care. For private pay clients, payment terms will be discussed and agreed upon during the initial consultation. Accepted payment methods include check, credit card, and electronic transfer. There are no charges for initial consultations or assessments.
3. Scheduling, Cancellations & Changes
Services are scheduled based on mutual agreement between the patient/family and our clinical team. We make every effort to maintain consistent scheduling, but adjustments may be necessary due to patient needs, staff availability, or emergencies. We ask that patients or families provide as much notice as possible if a visit needs to be rescheduled or canceled. In turn, we will notify you promptly of any changes on our end. In the event of severe weather or other unforeseen circumstances, we may need to reschedule visits for safety reasons.
4. Client & Family Responsibilities
Patients and families are expected to treat our staff with courtesy and respect. Any abusive, threatening, or discriminatory behavior may result in termination of services. Accurate and complete information about medical history, current conditions, medications, and special needs must be provided to ensure safe and effective care. Patients and families are responsible for maintaining a safe home environment for our caregivers (e.g., smoke-free during visits if requested, secure pets, clear pathways).
5. Privacy & Data Protection
We collect and store personal health information securely in compliance with HIPAA and all applicable state and federal laws. Your information will only be shared with members of your care team, authorized insurance providers, or as required by law. We will never sell or share your data for marketing purposes. 6. Liability & Service Limitations
While our team is highly trained and dedicated to providing excellent care, outcomes can be influenced by many factors beyond our control, including underlying health conditions and disease progression. Pulse Home Health & Hospice is not liable for health complications arising from pre-existing conditions or circumstances outside the scope of our services. We carry professional liability insurance and maintain all required licenses and certifications.
7. Modifications to Terms
Pulse Home Health & Hospice reserves the right to update these Terms at any time. Changes will be posted on our website with an updated "Effective Date." Continued use of our services after changes are posted constitutes acceptance of the revised Terms.
8. Governing Law
These Terms are governed by the laws of the State of Louisiana and applicable federal regulations.
